Pellet Therapy

Compressing or crystallizing the pure hormone powder of estradiol or testosterone produces a hard little rice shaped pellet. When placed under the skin, the concentrated pellet acts like a tiny ovary or testicle releasing life-enhancing hormones slowly into the bloodstream. The main advantage of pellet hormone replacement therapy is a long-lasting stream of hormones that sustain the body to mimic the constant hormone release of a young adult.

I have performed hundreds of these procedures in my office over the past twenty years. Men may receive hormone pellets of testosterone that last up to 5 months. The best pellets contain a larger amount of testosterone per pellet, so that the surface area, and thus the rate of dissolving, is slowed. The largest sized pellet for men is 300 mg. The procedure takes about 20 minutes and requires only a bandaid at the end. The skin is numbed up with lidocaine and then the pellets are pushed under the skin into the fatty layer. With careful attention, the pellets should not be palpable, even in a slender individual. The pellets are placed just above the “back pocket” area on the buttocks.

All women could benefit from testosterone pellet therapy, but not all women benefit with estradiol pellet. It is my observation that women with a uterus should not receive estradiol as a pellet, since to give enough estradiol to sustain the bones, brain, skin, and vagina, the initial dose may stimulate unwanted uterine bleeding. For women who have not had a hysterectomy, estradiol is best replaced by a cream or a tablet each day as a precise dose. Other doctors give a tiny estradiol pellet of 10 or 12 mg, but the blood level will be sub-therapeutic and symptoms of estradiol lack, such as hot flashes and dry vagina, may result.

I have observed in my patients that libido is most increased by the testosterone pellet. Also, few women revert back to a daily cream application after enjoying the freedom of sustained boost of testosterone.
For women who have had a hysterectomy, both estradiol and testosterone may be given in a combined pellet or two separate pellets at the same time.
